A standard condition of Pretrial Supervision is for all supervised defendants to report as instructed by their respective pretrial services officer. Defendants may be required to submit a monthly supervision report (MSR). Below is a link to the pdf version of the MSR that defendants may print and submit via US Mail.
Please note:
MONTHLY REPORTS AND ACCOMPANYING DOCUMENTATION (such as pay stub verification) ARE DUE WITHIN THE FIRST TEN DAYS OF EACH MONTH.
MSR's may be filled-in via a computer, but they must be printed and signed to be considered valid. No digital signatures are allowed at this time.
